- Alleged Perpetrator 2 (AP2) is a contracted worker that does various tasks for different residents. AP2 has access to service plans but is not required to read them, they are not always accurate and relies on regular facility staff for updates and/or to inform AP2 of specific resident needs. On or about February 2, 2020, AP2 assisted Alleged Victim (AV) to use the restroom. AV tripped on his/her ambulation device, fell obtaining a mark on his/her head. AV's care plan states he/she is independent with ambulation, however the care plan list multiple supports for AV for staff to assist AV with ambulation. The facility failed to assure a qualified caregiver was present, which is a violation of resident rights, is neglect of care and constitutes abuse.
