Findings
The facility failed to implement effective interventions and appropriately monitor W1 and the Alleged Victim (AV) according to their aggression and prior altercations. The failure resulted in a physical altercation and W1 hit AV in the face causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
