Findings
On or about February 8, 2020, Alleged Victim (AV) had a fall that resulted in AV suffering fractures to his/her L2 vertebrae, a head injury, and massive bruising. An investigation determined that AV had approximately eight (8) documented falls between January 11, 2020 and February 8, 2020 with no changes to AV's care plan until the February 8, 2020 incident. The facility's failure to properly plan AV's care and to implement interventions to reduce AV's fall risk is a violation of resident rights, constitutes abuse, and is considered neglect of care.
