Findings
The facility failed to provide oversight and appropriately care plan for Alleged Victim (AV) regarding his/her fall risks. In November 2019, AV experienced 4 falls, with 2 resulting in injury. AV was care planned to have a seat belt alarm to alert staff when he/she was getting up. The facility failed to follow AV's care plan when on or about November 26, 2019, AV was seated in a chair without his/her seat alarm and experienced a fall. AV's care plan was not updated until January 2020 to reflect AV's fall risk. The facility's failures are a violation of resident rights, are considered neglect of care and constitutes abuse.
