Findings
On or about January 29, 2020, Alleged Victim (AV) was found to have skin discoloration on the back side of his/her upper right arm. An investigation determined that facility did not know what caused AV's skin discoloration/bruise. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care, and constitutes abuse.
