- On or about February 11, 2020, Alleged Perpetrator #2 (AP2) gave the Alleged Victim (AV) incorrect medication. AV did not have any adverse side effects, however, was transported to the hospital as a precautionary measure, as it was noted AV was allergic to aspirin. After this incident, AV's doctor sent in a change to AV's record to remove aspirin as an allergy. AP2 failed to properly administer AV's medications, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The Facility failed to provide a safe medication administration system which is a violation of Oregon Administrative Rules.
