Findings
The facility failed to provide a safe medication administration system to ensure physician orders were followed when Alleged Victim (AV) was released from the hospital on February 8, 2020. AV was discharged back to the facility with a new physician's order to discontinue his/her blood thinner medication for two (2) days. An investigation determined that AV did receive his/her blood thinner medication on February 8, 2020 for his/her pm dose, on February 9, 2020 for all doses, and on February 10, 2020 for his/her am dose. The facility's failure to follow AV's physician's orders exposed AV to potential risk of harm which is a violation of resident rights, is considered neglect of care and constitutes abuse.
