Findings
The facility failed to provide a safe environment on or about February 8, 2020, when the Alleged Victim experienced a cabinet door falling on his/her foot. The failure resulted in AV being transported to the doctor and was found to have two fractured toes, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
