Findings
The facility failed to appropriately care plan and provide services to the Alleged Victim (AV) according to his/her needs. The failure resulted in AV going approximately two months without needed services causing his/her medical condition to continue and worsen, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
