On or about 02/13/20, Alleged Perpetrator 2 (AP2) grabbed Alleged Victim's (AV) right arm while assisting AV in the dining room, leaving a red mark on AV's right arm. AP2's actions are considered physical abuse. The facility failed to provide a safe environment which violates Oregon Administrative Rules.
