Findings
On or about February 16, 2020, Alleged Perpetrator #2 (AP2) wrongfully restrained the Alleged Victim (AV) in his/her wheel chair by setting the brakes are placing a gait belt on him her. AP2's actions are a violation of resident rights and dignity, is considered neglect of care and constitutes abuse. The facility's failure to protect AV from loss of dignity is a violation of Oregon Administrative Rules.
