Findings
On or about February 16, 2020, a visitor to the facility accidentally allowed for the Alleged Victim to exit the locked facility without assistance. The Alleged Victim was found about an hour later by law enforcement across the street at a market. According to documentation, there was no injury to the Alleged Victim. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
