Findings
Witness 1 (W1) has a known history of aggressive behavior and just prior to this incident was aggressive with another resident. On or about February 16, 2020, W1 hit Alleged Victim (AV) causing a bruise on AV's thigh. The facility failed to provide a safe environment which is a violation of resident rights, is considered neglect of care and constitutes abuse.
