- On or about February 18, 2020, Alleged Victim (AV) fell out of bed and started calling for help. A visitor of the facility responded and got staff. Staff put AV back in bed. Staff reported feeling ill and did not complete a proper assessment of AV after the fall and did not document the fall so AV could be monitored. AV care plan does not reflect AV's current care needs and has much conflicting information. Day shift staff found AV with bruises on his/her face and was transported to the emergency room for medical treatment. The facility failed to care plan appropriately to mitigate AV's risk of injury due to falls and the facility failed to respond to AV's change of condition, which is a violation of resident rights, is neglect of care and constitutes abuse.
