- Alleged Victim (AV) has a condition that requires assistance from facility staff. On or about February 20, 2020, Adult Protective Services received a self-report alleging that on February 19, 2020, a bruise was discovered on AV's lower rights back/buttock area and AV reported no pain in the affected area however per progress notes AV stated it was tender to the touch. Additionally progress notes state the bruise was discovered on February 17, 2020. During the interview process it was reported that AV is independent with mobility, does not use assistive devices and does not require much hands on assistance with activities of daily living. AV's service plan states that AV requires complete assistance with activities of daily living. Progress notes indicate on several different occasions AV was showing signs on exit seeking behavior however AV's service plan states AV does not have any behaviors. The facility failed to care plan according to AV's observable needs, which is a violation of resident rights, is neglect of care and constitutes abuse.
