Findings
The Alleged Perpetrator 2 (AP2) failed to provide safe medication administration to ensure the Alleged Victim's (AV) was given the correct medication. The failure resulted in AV receiving the wrong medication and was transferred to the hospital for evaluation, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to provide a safe medication administration system, which is a violation of Oregon Administrative Rules.
