Alleged Perpetrator 2 (AP2) purposely threw water in the Alleged Victim's (AV) face to punish him/her for acting out. AP2's actions are considered physical abuse, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to protect AV from physical abuse, which is a violation of Oregon Administrative Rules.
