Findings
The facility failed to appropriately care plan for the Alleged Victim after he/she suffered an injury to his/her back on 12/24/2019. On or about 2/18/20, AV requested help with his/her shower, Alleged Perpetrator #2 (AP2) answered the call light and AV was taken aback about an opposite sex caregiver giving him/her a shower. Having a shower by an opposite sex caregiver created emotional harm to AV. The facility failed to update AV's care plan to his/her preferences regarding help with showers and other cares. The investigation determined no wrongdoing or abuse on the part of AP2. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
