Findings
On or about February 23, 2020, Alleged Victim (AV) eloped from the facility when a wind storm knocked power out multiple times. AV was found unsupervised by a facility staff member in the facility parking lot. The facility failed to provide a safe environment for AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
