Findings
Alleged Victim (AV) is a known fall risk and requires staff assistance with mobility. Prior to the incident AV had increased attempts to self-transfer and/or self ambulation related to increased urination. On or about February 25, 2020, AV attempted to ambulate without staff assistance and fell fracturing his/her finger and sustaining a concussion. The facility failed to intervene when AV's condition changed, which is a violation of resident rights, is neglect of care and constitutes abuse.
