Findings
Alleged Victim (AV) relies on the facility to provide care. At the beginning of NOC shift staff were made aware that AV had locked his/her door and staff were unable to locate keys. At approximately 1230am, staff heard AV yelling for help after falling inside his/her room. Staff could not provide assistance to AV due to not being able to access his/her room. Emergency Services were able to break down AV's door to gain access to AV's room. AV was locked inside his/her room from 9:50pm to 12:37am putting AV at risk for serious harm. The facility failed to provide a safe environment which is a violation of resident rights, is neglect of care and constitutes abuse.
