Findings
On or about February 27, 2020, The Alleged Victim (AV) experienced a non-injury fall while being transferred with the sit to stand by Alleged Perpetrator #2 (AP2). AP2 failed to follow AV's care plan by transferring AV alone. AV's care plan states he/she is a two person transfer with the sit to stand. AP2's actions are considered neglect of care which constitutes abuse. The facility failed to provide a safe environment which violates Oregon Administrative Rules.
