Findings
The facility failed to implement interventions and appropriately care plan according to the Alleged Victim's (AV) known fall history. The failure resulted AV experiencing approximately ten falls in about three months, causing AV to be transported to the hospital three times in four days, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
