Findings
On or about March 5, 2020, Alleged Perpetrator #2 (AP2) failed to recognize the Alleged Victim (AV) as a resident and allowed him/her to leave the facility without supervision, placing AV at a risk for harm. The AV left the locked facility and wandered the campus until found by another staff who escorted the AV until a staff could retrieve him/her. AP2's actions are considered neglect of care and constitute abuse. The facility's failure is a violation of Oregon Administrative Rules.
