Findings
The facility failed to implement interventions and appropriately care plan according to the Alleged Victim's (AV) known fall history. The failure resulted AV experiencing approximately forty falls in about four months, causing repeated unreasonable discomfort and injuries, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
