Findings
On or about March 3, 2020, Alleged Perpetrator 2 (AP2) spoke inappropriately to the Alleged Victim (AV) while performing a brief change, causing a loss of dignity to AV. AP2's actions are considered verbal abuse. The facility failed to protect the AV from verbal abuse, which is a violation of Oregon Administrative Rules.
