Findings
Alleged Perpetrator #2 (AP2) administered the incorrect medication to the Alleged Victim. On more than one occasion, AP2 has administered AV's spouses medication to him/her in error, place AV at a risk for serious harm. AP2's actions are considered neglect of care and constitute abuse. The facility failed to provide a safe medication administration system which is a violation of Oregon Administrative Rules.
