Findings
The facility failed to ensure supervision and staff support regarding known behaviors related to Witness #2 and the Alleged Victim. An incident occurred between the Alleged Victim and Witness #2 where Witness #2 had his/her hands around the Alleged Victim's neck, causing unreasonable discomfort and a risk of harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
