Findings
On or about March 8, 2020, Alleged Victim (AV) left the facility when it was unsafe for AV to be out of the facility without support. On or about March 11, 2020, AV's service plan was updated to provide one on one supervision to prevent elopement. On or about March 13, 2020 and March 16, 2020, AV successfully left the facility without support. The facility failed to follow AV's care plan, putting AV at risk for serious harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
