Findings: The facility failed to provide a safe environment for the Alleged Victim (AV). On or about March 15, 2020, it was requested by the Administrator not to let AV and W1 be alone in AV's room, however, on or about March 17, 2020, W1 was seen leaving AV's room very upset. On or about March 18, 2020, during a shower given by staff to AV, a large bruise was found on AV's thigh. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP20-01002 $188.00 fine assessed
