- Alleged Victim (AV) resides in a locked facility due to dementia. On or about March 21, 2020, Alleged Perpetrator 2 (AP2), disable the door alarm and let AV exit the unit and enter the assisted living building. AV was given a work phone and instructed by AP2 to give it to the first person he/she saw. Staff in the Assisted Living Building were unaware that AV was roaming around the building unsupervised. AP2 then went out of the memory care, located an Assisted Living staff member and told them they failed "the test" and then laughed. AV was located by a staff member and escorted back to the memory care. AP2's actions put AV at risk for serious harm, which is considered neglect of care and constitutes abuse. The facility failed to provide a safe environment which is a violation of Oregon Administrative Rules.
