Findings
The facility failed to provide a safe environment for the Alleged Victim (AV) to properly plan care surrounding bruising of unknown cause. AV had three bruises found on his/her inner arm in March and has had bruising in the past of unknown cause. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
