- Alleged Victim (AV) relies on facility staff to provide incontinence care and is at risk for skin breakdown. On or about March 21, 2020, Alleged Perpetrator 2 (AP2) was the assigned care provider to AV. AP2 reports the facility was short staffed and he/she asked other staff to provide care to AV. AP2 asked other staff if care was provided to AV and documented that incontinence care was provided to AV. At approxemetly 10:00pm, the next shift went to provide care to AV and noted the AV's brief was soaked and day clothes were still on, the date and time written on the brief was prior to 3:00pm. AP2 did not ensure incontinence care was completed, resulting in unreasonable discomfort and creating a risk of serious harm to AV. Some facility staff reported that only the assigned caregiver was to chart care such as incontinence care. The facility failed to have procedures in place to ensure cares are provided and documented appropriately and failed to ensure there was enough staff to meet the scheduled and unscheduled needs of AV which is a violation of resident rights, is neglect of care and constitutes abuse.
