- Alleged Victim (AV) requires assistance with transfers. On or around March 13, 2020, Alleged Perpetrator 2 (AP2) assisted AV with a one-person transfer into bed. The transfer happened more suddenly than AV was used to. After the transfer AV felt scared of AP2. Following the event AP2 informed management that AP2 would not be going into AV's room anymore, in order to provide cares, because AV didn't like AP2. Management did not follow up with AV to get more information regarding why AP2 would not be going into AV's room. AV stated that he/she filled out a grievance and placed it in the grievance box. The facility denied any knowledge of a grievance. The facility failed to provide a safe environment for AV which is a violation of resident rights, is neglect of care and constitutes abuse.
