- On or about March 16, 2020, Alleged Perpetrator 2 (AP2) was observed disposing of Alleged Victim's (AV) pureed meal without offering it to AV. AP2's actions caused AV unreasonable discomfort due to hunger. AP2's actions are considered neglect of care which constitutes abuse. The facility is responsible for the supervision, training, and overall conduct of staff when staff are acting within the scope of his or her employment duties. The facility's failure to provide a safe environment is a violation of Oregon Administrative Rules.
