Findings
On or about April 4, 2020, the Alleged Victim and Witness #1 had a non-injury altercation. The facility failed to implement meaningful interventions and appropriately care plan regarding Witness #1's behaviors, placing residents at a risk for harm. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
