Findings
On or about April 5, 2020, Alleged Perpetrator #2 (AP2) failed to follow the Alleged Victim's (AV) care plan to transfer him/her with the hoyer lift using two people. During AP2's transfer of AV, he/she fell and received a bruise. AP2's actions are considered neglect of care and constitute abuse. The facility failure is a violation of Oregon Administrative Rules.
