Findings
Alleged Perpetrator #2 (AP2) failed to follow the Alleged Victim's (AV) care plan to use a gait belt during transfers. On or about April 4, 2020, AP2 was assisting AV from a sit to stand position and did not use a gait belt as outlined in AV's care plan, which placed the AV at a risk for serious harm. AP2's action is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility's failure to provide a safe environment is a violation of Oregon Administrative Rules.
