Findings
Alleged Perpetrator 2 (AP2) failed to follow the Alleged Victim's (AV) care plan. AV was care planned as a 2/3 person transfer, however, on or about April 6, 2020, AP2 attempted to transfer AV alone and while transferring AV, he/she fell to the ground, fracturing his/her leg, requiring surgery. AP2's actions are considered neglect of care and constitute abuse. The facility failed to provide a safe environment, which is a violation or Oregon Administrative Rules.
