Findings
On or about April 16, 2020, it was discovered that the Alleged Victim (AV) had 3 bruises found on his/her arms. It was undetermined how the bruises happened, and staff failed to notify clinical staff of the bruises in a timely manner. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
