Findings
The facility failed to follow the Alleged Victim's (AV) care plan and provide supervision according to his/her needs. The failure resulted in AV leaving the facility unsupervised and was transported to the hospital exposing him/her to risk of serious harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
