Findings
The facility failed to provide a safe environment for the Alleged Victim (AV) as AV was found to have a bruise on his/her foot, of unknown origin. An internal investigation could not determine the cause of the injury to AV. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
