Findings
The facility failed to implement interventions and ensure all caregivers were aware of the Alleged Victim's (AV) care needs regarding his/her fall history. The failure resulted in AV experiencing approximately eighteen falls in about two months causing injury and repeated unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
