Findings
The facility failed to follow the Alleged Victim's (AV) care plan to have staff in the common area when residents are in there. AV fell, was transported to the hospital and returned with three staples in his/her head. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
