Findings
On or about April 25, 2020, the Alleged Perpetrator 2 (AP2) failed to provide Alleged Victim (AV) with incontinence care, leaving AV in his/her soiled brief for an extended period of time. AP2 failed to provide appropriate care to AV, resulting in unreasonable discomfort. AP2's actions is considered neglect of care which constitutes abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
