- On or about April 5, 2020, Alleged Perpetrator 2 (AP2) failed to secure and keep hazardous materials out of Alleged Victim's (AV) reach when AP2 was cleaning the kitchen. AV reached over the counter, unscrewed the lid of the spray bottle of chemicals, and took a drink of the hazardous chemicals. AP2's actions are considered neglect of care and constitutes abuse. Following AV’s ingestion of cleaning chemicals, and prior to reporting AV’s chemical ingestion, Alleged Perpetrator 3 (AP3) provided AV a glass of milk, and instructed AV to drink the milk, which AV did. AP3’s instructions to AV to drink milk immediately after AV ingested chemical solution were not aligned with instructions provided to the facility by Poison Control and On-Call Physician, who instructed facility staff to rinse AV’s mouth. AP3's actions are considered neglect of care and constitutes abuse. The facility failed to provide a safe physical environment for AV which is a violation of Oregon Administrative Rules.
