- Alleged Victim (AV) is at risk for dehydration and his/her family was providing AV coconut water regularly in the evenings. Starting March 11, 2020, the family was no longer able to visit AV due to COVID-19 precautions. The family provided the facility with coconut water to give to AV in the evenings and AV's service plan was updated to give AV eight (8) ounces of coconut water before bed. Facility records indicate AV was given coconut water daily between March 11, 2020 and April 20, 2020. However, facility audit determined the amount of coconut water remaining did not reconcile with daily administration of the coconut water. On or about April 20, 2020, AV was hospitalized and diagnosed with a urinary tract infection and dehydration. The facility failed to manage the administration of coconut water appropriately resulting in AV being dehydrated and hospitalized, which is a violation of resident rights, is neglect of care and constitutes abuse.
