Findings
On or about May 2, 2020, the Alleged Victim (AV) attempted to take his/her own life by taking an overdose of over the counter medication, which he/she had in her possession at time of move in at the facility. AV had a history of self-harm and had previously been living in a psychiatric facility. Upon move in, the facility failed to inventory AV's items to ensure his/her safety, which placed AV at a risk for serious harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
