Findings
The facility failed to follow the Alleged Victim's (AV) care plan to to have a tab alarm, his/her bed was in the lowest position and fall mat in place while AV was in bed. AV had multiple falls from April 29, 2020 to May 1, 2020 and it was found the tab alarm either wasn't on or wasn't working and the fall mat was under the bed. AV suffered an abrasion to his/her knee. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
