Findings
The facility failed to provide appropriate services according to the Alleged Victim's (AV) needs and change of condition. There is no documentation that the facility followed AV's temporary service plan to push fluids which resulted in AV being sent to the hospital for possible dehydration, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
